The start of a New Year is an excellent time to schedule your annual exams and/or preventative screenings. Annual exams and preventive screenings can detect issues before they become chronic conditions.
As part of the SmartHealth Medical Plan, below are some of the preventive services offered:
Annual Routine Physical
Well Baby/Child Care
Routine Immunizations
Well Woman Preventative
Women 10 years or older can receive two annual exams, which include
One well woman visit with a primary care provider;
One gynecological visit, and/or;
One natural family planning visit.
Annual Mammogram
One baseline screening for females ages 35-39 years
One mammogram annually for females ages 40 and over
Screening Colonoscopy
One screening every 10 calendar years for those ages 50-75 years

These services are covered at no cost to you and your covered dependents when you receive your medical care from an Ascension Network or National Network provider (physician or facility). If you use out-of-network providers, you'll pay 50% of the cost after you meet the deductible. By completing your annual well visit with your primary care provider, you’ll also earn $125 in Healthy Rewards for the SmartHealth Well-being program.
